Another test is the coronavirus antigen test. To carry it out, a smear is taken from the nose or the back wall of the pharynx (as for a PCR test), the result can be obtained after 10-15 minutes. Antigen tests have a very high specificity (up to 100%), so false positive results are rare. But the sensitivity of these tests is insufficient - from 84% to 97%, which is manifested by a large number of false negative results.
The next, largest group in terms of the number of tests registered in Russia for the determination of coronavirus is tests for the determination of antibodies.
For reference: antibodies are protein molecules (immunoglobulins) produced by blood B-lymphocytes in response to foreign agents entering the body, in our case, the SARS-CoV-2 coronavirus. Antibodies to various pathogens of infectious diseases are specific and their presence in the blood gives information about the presence of infection and even about the course of the disease (acute or advanced phase).
The test determines two types of antibodies - IgM and IgG immunoglobulins.
IgM immunoglobulins begin to be produced in response to infection, appear in the acute phase of the disease (5-7 days after the onset of the disease); IgG immunoglobulins appear in the blood 10-14 days after the onset of the disease. They provide the immune response (humoral) of the body to fight infection.
Thus, if IgM(+) is determined as a result of the test – the patient is in the acute phase of the disease, IgG(+) is the subject or has had a coronavirus infection and has developed an immune response or, in the presence of clinical manifestations of COVID-19, he has an advanced stage of COVID-19.
The subspecies of antibody tests are enzyme immunoassay (ELISA) and immunochromatographic analysis (IHA). For enzyme immunoassay (ELISA), as well as for PCR, a laboratory is required, the time to obtain the result is 1-2 days. In addition, venous blood sampling is required for ELISA. The above, despite the very good specificity and sensitivity of ELISA, makes it impossible to use this type of tests for rapid testing for coronavirus.
